Application Forms

Application forms may be requested from the Registrar's office or downloaded from this web site. The completed form should be returned with other required documents before January 31st of the year in which admission is sought.

Applicants seeking entry into the Master of Divinity program must also submit a completed ecclesiastical recommendation form and an academic recommendation form, to be completed by a recent post-secondary instructor, along with the completed application form.  Click on the forms below to print out a copy:
